I’m confused. I clicked on your link and fill out my info, I clicked the ‘get permission’ link at the bottom and when it goes to that page it is blank, but it won’t let me complete the form without it. Any suggestions?
I tried in both Google Chrome and Internet Explorer

Yes, there’s an issue with facebook and the sites application for th free sample which makes it impossible to fill out the form w/facebook.
