This is the Great American Flu Shot Challenge!  Print out your Rebate Form to get your $5 mailed to you!  You do not have to buy the LYSOL and get the flu shot at the same time.  You can do them at different times as long as it falls into the valid dates listed below.

Even if the flu shots were covered by your insurance, just keep a receipt from them that you can mail in.  AND you can buy ANY Lysol product so go through your receipts between October and now and I’ll bet you have one.  It doesn’t matter where you bought the Lysol or got the shot!the shot!

Here are the Terms and Conditions:

• Receive $5.00 by mail when you purchase 1 LYSOL product and a flu shot.  Offer is valid and receipt(s) must be dated between 10/1/11 and 2/29/12.
• Mail the original, dated receipts showing purchase of 1 LYSOL product and payment for the flu shot.  Print your full address clearly for proper delivery of your Mail-In Offer.
• This offer is good in the US, excluding Rhode Island.  Limit of one offer submission per name, address or household.  Allow 4 – 6 weeks for delivery.
